If you are over age 60 in Cook or Lake County and live independently at home, our team of care coordinators is available to help. They will visit you in person at your home, assess your needs, and connect you with in-home services that will help you continue to live safely, independently, and with dignity in your community.

We work with individuals, families, and community partners to develop a plan of care to assist seniors with daily activities in the home and community. Our care coordinators provide person-centered, in-home assessments to determine the level of care seniors need to remain safely in their homes, with the goal to help seniors age in place, living independently, or with caregivers for as long as possible.
Services offered:
- Assessment for Community-Based Services
- Access to In-Home Service, Adult Day Services, Emergency Home Response Services, and Automated Medication Dispenser Service
- Access to Home-Delivered Meals, Chore Housekeeping, Caregiver Services, and Benefits Assistance
- Care Plan Development and Monitoring
- Nursing Home Pre-Screening
