If you or your loved one is living with a long-term disability, affordable housing and support are available to help you live safely and independently in the community.

Pope John Paul II and Hayes Manor are permanent affordable housing communities in Cook County serving people with disabilities. A full-time care coordinator is on-site to help you access services you may need.

Long-term support is also available through the Bridge Rental Subsidy program for eligible people with disabilities or serious mental illness who seek assistance transitioning from long-term care settings to independent living in the community.

Affordable housing: We offer apartments designated for persons with verifiable disabilities and requiring features of an accessible apartment at Pope John Paul II and Hayes Manor. 

One or more person in the household must be at least 18 years or older and have a disability of long-continued and indefinite duration that substantially impedes the person’s ability to live independently. Households must meet income requirements as determined by the U.S. Department of Housing and Urban Development. 

All rents are subsidized. Eligible households pay no more than 30% of their adjusted gross income for rent and utilities, excluding telephone. Certain deductions are made for elderly status for all household members. After initial occupancy, income will be recertified on an annual basis.

Applications are available in person at the front desk of the properties or will be mailed after calling the properties. 

 

Bridge Rental Subsidy Program: The Illinois Department of Human Services Division of Mental Health refers those who are eligible. Contact the Illinois Help Desk at Maximus: (833) 727-7745, select option #4 to learn more about the program and eligibility.
